For Poly PT, there is no compulsory subject requirement.  But it has a list of preferred subjects with weightings that will be applied in calculating the admission scores.
For 2024 entry, the highest subject weightings are Chin, English, Biology & Physics.  
The scores of these subjects will be multiplied by a factor of 10.   
The subject weighting for Chemistry is only 7.

I attended the school's presentation some time ago.  I remember that the school must submit a 學校評核報告 (school reference report SRR) to JUPAS for its student.  The SRR includes the academic results of the student, in a format similar to what you describe in above post.
