Can I have one too?? 我download 咗份FORM, 但只係開到個學校名同REF NO. , 下面要填嘢嗰PART開唔到呀!!! 或者有冇好心媽咪可以借份FORM 我COPY 返下面嗰PART???? 我可以比我個REF NO.你睇返, 唔會同你嗰張FORM 混淆架!!! Milliion Thanks AR!!!

Hi, princessvivi !

I have sent some post to ask for the application form, but no one send me yet. Actually I have downloaded it by myself already, but don't know why I can only open the school name and my reference number but not other area (the area to fill in my information)!! It will be very kind of you if you can send me the form below (without reference number is ok) so that I can add it with my own reference number.
I've called the school but they said no method to send me the softcopy again!

Really million thanks if you can help me!!
